Title: The Role of GPs & NPs in Advance Care Planning

Overview:

With increased emphasis on Advance care planning (ACP) from government and health organisation and the aim to increase health literacy of patients and residents, it is important to recognise that challenges exist in supporting community expectations that individuals have a right to autonomous decisions when considering health and personal care. Health professionals understand they play an important role and understand that these ACP is an iterative process and should be regularly reviewed. Health professionals are aware that WA have a new Advance Health Directive and that there are processes, laws, and statutory documents involved in ACP.

This workshop outlines your role and how you can appropriately care for your patient or resident and encourage the uptake of AHD or Values and Preferences forms and the benefits for them, their loved ones or carers and the health sector.

Learning Outcomes:

1. Identify the WA Advance Care Planning documentation flow chart.

2. Identify how you can imbed ACP into your practice.

3. Identify the benefits of an AHD, Values and preferences form, RGOC and your legal responsibility.
