Title: Recognising Deterioration in Residents of Aged Care Facilities
Overview:
GPs and NPs play a special role in health and see residents and patients regularly over the course of their latter years. Recognising and acknowledging deterioration of a resident in an aged care is a vital clinical tool.
Accurate recognition allows for the resident or patient to have exceptional personalised care and staff prepared to care according to their wishes. This can be communicated with facility staff, family and carers. Staff are then able to prepare to care with reviewed patient wishes and goals of care, place of care and required support, medications are prescribed and deprescribed, families are aware of care and symptoms. Lastly it can assist with futile hospital or medical interventions.
Learning Outcomes:
1. Identify deterioration in a resident/ patient.
2. Identify steps required to prepare facility, staff, resident and family for periods of deterioration, terminal phase and last days of life.
3. Identify appropriate palliative and end of life care tools.
