Title: Advance Care Planning (ACP) and Residential Goals of Care (RGoC) in Aged Care Facilities

Overview:

With increased emphasis on Advance care planning (ACP) from government and health organisations and the aim to increase community health literacy, it is important to recognise that challenges exist in supporting community expectations that individuals have a right to autonomous decisions when considering health and personal care.

RACEPC Communicate and PaSCE have ACP specific courses which are highly recommended for facility staff; however, this session focuses on the Residential Goals of Care (RGoC) form and its use and purpose. A RGoC does not replace an Advance Health Directive (AHD) or Values and Preferences form, it is applied in combination with.

This workshop clarifies the role facility staff play and how they can embed the form into practice, so it benefits not only residents, but the facility staff and allows appropriate care of residents.

Learning Outcomes:

1. Identify the WA Advance Care Planning documentation flow chart.

2. Identify how you can embed ACP & RGoC into your practice.

3. Identify the benefits of an AHD, Values and preferences form, RGOC and your legal responsibility.

MPaCCS is a metropolitan mobile specialist palliative care team that works collaboratively with General Practitioners and other health professionals to ensure superior outcomes. They can assist the following facilities:

• Mental health and psycho-geriatric.

• Residential aged care.

• Residential disability.

• Aboriginal and Torres Strait Islander medical service.

MPaCCS also provide a palliative care consultancy service to the Pilbara in collaboration with WACHS.
